  IS YOUR LOVED ONE too far away?  Karen can help with long distance relocation of your elder relative. Travel arrangements, transfer of records, preparing the elder for the move, and dealing with moving personal property are all tasks that can be difficult if you live in another city. Karen can also accompany your elder or you and your elder on the trip.

Bill Paying Service
  IS KEEPING TRACK of finances getting to be too much for your elderly relative? Karen, and her associate Debra Benson, can assist elders with household financial management - paying bills, balancing the checkbook, developing a budget, keeping to a budget.

The Bill Paying Program can include -

Benefits Assistance Program

  • Assistance in applying for Medicare, Medicaid and Veterans Benefits
 
     
Nursing Home Placement Program
  FOR SOME ELDERLY family members the only care option is nursing home placement. This is not a simple process. A bewildering number of choices and obstacles will arise as soon as you take the first steps toward placement. Karen has many years of experience in advocating with the area nursing homes and medical providers and can help you get the placement that is best for your loved one.

The Nursing Home Placement Program Provides -

  • An in-home assessment of the needs of the elderly person and of the family.
  • Arrangements for a Patient Review Instrument (PRI) and screen to be performed to determine the level of care that is needed.
  • Recommendations of area nursing homes and discussion of financial issues.
  • Assistance in obtaining and completing applications for the desired nursing homes.
  • Advocating for the family's choice of placement.
  • Assistance on moving-in day.
  • Follow-up as needed.
 
     
Alternate Living Placement Program
  OFTEN AN ELDERLY relative cannot care for themselves at home anymore but do not meet the medical requirements for admission to a nursing home. In a case like this Karen can help a family explore the many Assisted and Enriched Housing options that are available in the Rochester area. A resident of an Assisted Living or Enriched Housing facility of has his or her own apartment but extra services such as meals, housekeeping and medical assistance are available. Often such facilities provide progressive levels of care so that an elderly person can have more help when they need it and yet have future changes in their living situation kept to a minimum.

The Alternate Living Placement Program Provides -

  • An in-home assessment of the needs of the elderly person and of the family.

Independent Living Program
  TO HELP SUPPORT the elder who can continue to live at home but needs increasing assistance with life's details, Karen has an Independent Living Program. A primary role for the Elder Care Manager in this program is to recommend, arrange for and supervise care providers that come into the home.

The Independent Living Program Provides -

  • An in-home assessment of the elderly person's physical and emotional needs.
  • Coordination and monitoring of medical care and in-home care. This includes transportation arrangements and assistance with communication with healthcare providers.
  • Referrals to reliable financial and insurance specialists in the area.
  • Encourage quality family and social involvement which may include participation in senior center programs, support groups, adult day programs, holiday celebrations, etc.
  • Periodic written status reports to the family.
  • 24/7 availability to help deal with emergencies
 
     
Caregiver Support Program
  IF YOU HAVE CHOSEN to be the primary caregiver for your aging or disabled relative, you already know that this can be a rewarding, yet consuming job. Many caregivers find that as time goes by the stresses of caring for a loved one begin to outweigh the joys. This may mean that you need to make changes in your life that will give you more support as a caregiver. Karen can help relieve the strain of caregiving by becoming a part of your support system.

The Caregiver Support Program Provides -

  • An in-home assessment of the caregiver's needs
  • A monthly visit from Karen
  • A weekly check-in call from Karen
  • 24/7 availability to help the caregiver deal with the emotional and physical strain of an ever-changing situation
